Insurance and Rates Information
Insurance
We currently accept Blue Cross Blue Shield PPO plans, Cigna PPO, United Behavioral Health/Optum, select Employee Assistance Programs (EAPs), Aetna, and Medicare. If you do not see your insurance company listed or do not have insurance, please contact the front desk to see what payment options are available to you.
Insurance Rates
Insurance coverage and rates vary by insurance provider, as well as your specific insurance plan. Benefits verification and an estimate of cost for sessions is available upon request. Please let the front desk know if you would like your benefits verified.
Payment
Payment for treatment is due at the time services are rendered. Payments may be made by cash, check, or credit card. You may put a credit card on file for any charges not covered by insurance. Services may be terminated due to non-payment. Any payment received from insurance companies will be promptly credited to your account. This office does file for secondary insurance if you have Medicare primary insurance; please bring both insurance cards to your first appointment so that your provider can obtain copies of the cards.
Cancellation Policy
Once an appointment is scheduled, you will be expected to pay for this time. If you choose to cancel your appointment, we require 24 hours advance notice. A $75.00 fee will be charged for cancellations with less than 24 hours notice. If you fail to show up for an appointment you will be charged $130.00.
Insurance companies do not provide reimbursement for cancellations or no show appointments. This will be your responsibility. Please note that service may be terminated due to multiple missed appointments.
